Senior medical students enrolling in one of the Family and Community Medicine courses (research, independent study, preceptorships, subinternships) need to complete various forms that assist us in arranging and evaluating your clinical experience. In this way we document and confirm for the Student Records Office that students are enrolled in and complete an approved course of study.
Non-University of Arizona medical students should read special instructions about visiting students.

Fill out an Elective Selection Form completing Part IV: Goals and Objectives at the time you register for the elective. You do NOT have to fill out this part of the form if you go to Whiteriver Service Area IHS
FCM 891A/891B/891P: Submit a Report Form on your preceptor experience at the end of the rotation. You do NOT have to fill out this form if you are taking FCM 891Nz at Whiteriver.

FCM 811 (Subinternship):
Contact the subinternship coordinator at the site of your choice to confirm the weeks you will serve. UA students have preferred status until March 1. These sites contract with visiting students so reserve your spot early.

Submit a signed Approval Form to Phoenix Programs Office or to FCM Predoc Office (FCM room 206) at the time of registration.
FCM 815 ("specialty courses") and FCM 896 (seminar course):
Contact the instructor before enrolling. They will know if there are any changes or limitations associated with the course. Telephone numbers are included with the course descriptions in the electives manual.
International Health "Triad"

A hands-on clinical experience on any of the three Developing World continents arranged by you and the FCM faculty. Any 6 to 12 week period can be arranged (8 weeks is usual). Here you put your FCM896A preparation into practice - and get to do more clinical care and community medicine than you‘ve ever had responsibility for before.
3. Students who complete both FCM896A and FCM891Fi are eligible for consideration for the Mulcahy International Health Scholarship and/or the FCM Student Stipend . You get a stipend of approximately $200-240 per week (when funds are available), half paid in advance of your preceptorship departure if you meet all criteria.
