Year IV Electives

Senior medical students enrolling in one of the Family and Community Medicine courses (research, independent study, preceptorships, subinternships) need to complete various forms that assist us in arranging and evaluating your clinical experience.  In this way we document and confirm for the Student Records Office that students are enrolled in and complete an approved course of study.

Non-University of Arizona medical students should read special instructions about visiting students.

ALL Forms are available in Tucson (Room 206 FCM and in the Student Records Office) and in Phoenix at the Phoenix Programs Office (Kelly Lynch)
