Application Materials:

Complete this checklist and send all application materials to Candace Johnston (see contact information above) via email, facsimile, or US mail.

• A completed residency application form. The form can be downloaded from: http://www.gme.medicine.arizona.edu/applicants/forms/fellowshipapp.pdf
• A curriculum vitae, 3 pages maximum.
• A one page statement of interest.
• Three letters of recommendation from professors, supervisors, or individuals qualified to comment on past performance and ability to complete preventive medicine training. If you are currently a resident in or have completed a clinical training program, one letter must be from your program director.
• Copy of all college and graduate degrees including a M.P.H. if completed.
• Copy of graduation certificates from internship and/or residency training programs.
• Copy of USMLE certifications of completion of all parts of the USMLE.
• Copy of all state medical licenses.
• Copy of ECFMG certificate, if applicable.
